Nevermind.
My long post described an issue that didn't make sense, unsolvable problem.
I drove myself crazy, in the end, issue was with Fluxtone speaker I use for testing. The amp was working fine the whole time.

So if you test the signal output of that rca jack separately does the on / off issue still take place?
